About these templates
Run a short, regular survey on workload and engagement, read the answers fairly, and turn them into one or two actions your team can see.
Who it is for
Small-business owners who want an honest picture of how their staff are doing.
Managers and shift leads with a team of five or more who want a fair way to hear concerns.
HR professionals supporting several managers who need a simple, repeatable survey.
Anyone leading a change who wants to know whether it is working.
The problem it solves
Surveys often ask the wrong questions, make people worry about being identified, and produce results nobody acts on. This builder shows you how to protect anonymity, choose eight to twelve clear questions from a tested bank, read results without over-reading small differences, and run a short results meeting that ends with one or two actions. The Excel workbook does the counting for you.

Frequently asked questions
Can I use this with a very small team?
A survey cannot be anonymous with fewer than five people. The guide explains how to use the same questions as a structure for a team conversation instead.
Do I need survey software?
No. You can use paper forms and a sealed box, or any online form that does not record names. The Excel workbook handles the results.
How often should we run it?
Every two to three months, keeping most items the same so you can compare.
